In US, most Ph.D programs have two requirements: course work and thesis defense; and many programs have two exams to set milestones, qualification exam and comprehensive exam.
Taking an example, one guy adimitted by a ph.d program, s/he must pass qualification exam in the first one-two years, in this period, the majority of goal is course work, about 60 graduate credit hours to pursue, translate to courses is about 20 courses to be taken. In US universities, there aren't courses only for phd; there are for all graduate level students, that is, master's and phd's have classes togather. In addition, many graduate courses are open for undergraduate seniors, these credit can be transfered to graduate study if s/he pursue higher degree in the same school; inter-school credit transfer is also possible, but sometimes have a 9-12 credit hour upper limit.
After qualification exam, one has the Ph.d candidency. since then, s/he involved in research works from supervising faculty member. In this session, few course work is needed, s/he may take some classes just for fun or for knowledge needed. The main goal is doing research and publishing paper.
The research session may vary from 2 to 5 years, maybe even longer, depending on supervising faculty and his thesis subject. At the end of research session before thesis defense, some school require a comprehensive exam, normally, oral exam is involved too.
Given the story you told, this guy had taken most graduate level class when he was a undergraduate student and passed qualification exam, this two year is only for research; I think that he was very likely already had a master's degree, since it looks that his job was very related to his ph.d thesis, otherwise, he's a superman, not human; and, more important, his company was very cooperative, since many companies allow very few publicity of their R&D.
I have no intention to discredit his achievement, he is probably a genius, but a degree program is a kind of routine; it needs patience. I remotely remember that a record in US about Ph.d in engineering is 27 month; and for math is about 24 month.
